Eligibility

If you are scheduled to work at least 30 hours per week, you are eligible for benefits on your date of hire.

As you become eligible for benefits, so do your eligible dependents. In general, eligible dependents include:

  • Your spouse or partner: This includes your legal spouse or domestic partner.
  • Your child(ren): This includes your children to age 26, regardless of student, marital, or tax-dependent status (including a stepchild, legally-adopted child, a child placed with you for adoption, or a child for whom you are the legal guardian), as well as children of any age who are physically or mentally unable to care for themselves.

Enrollment

You can only sign up for benefits or change your benefits at the following times:

  • Within 30 days of joining Southwest Human Development as a new employee.
  • During the annual benefits enrollment period.
  • Within 30 days of a qualifying life event.

The choices you make at this time will remain in place through December 31, 2026, unless you experience a qualifying life event. If you do not sign up for benefits during your initial eligibility period, you will not be able to elect coverage until the next open enrollment period.

Know Where to Go for Medical Care

When facing an unexpected medical condition, you may think of going to the emergency room (ER) first. But this may not be your best option. At the ER, true emergencies are treated first, and other cases must wait—sometimes for hours. And it will cost you more to get care than at your physician’s office or an urgent care center.

Here’s a good place to start: If you’re not sure where to go, call your primary care physician (PCP) first. Your PCP can help direct you to the best place to receive care based on the urgency of the medical problem. In the case of a true medical emergency, call 911.

Frequently Asked Questions

Can I Change My Benefits During the Year?

Due to IRS regulations, once you have made your elections for the 2026 plan year, you cannot change your benefits until the next annual open enrollment period. The only exception is if you experience a qualifying life event. Election changes must be consistent with your life event.

Qualifying life events include, but are not limited to:

  • Birth or adoption of an eligible child.
  • Marriage, divorce, or legal separation.
  • Spouse’s work status changed affecting their benefits.
  • Death of your spouse or covered child.
  • Child’s eligibility for benefits changed.
  • Qualified Medical Child Support Order.

To request a benefits change, please contact a benefits counselor using the QR code above or update directly through UKG within 30 days of the qualifying life event. Change requests submitted after 30 days cannot be accepted. You may need to provide proof of the event, such as a marriage license or birth certificate.

How Can I Save Money with Tax-Free Accounts (FSAs and HSAs)?

Flexible spending accounts (FSAs) and health savings accounts (HSAs) let you set aside money before taxes for health care expenses making your dollars go further.

For 2026, the contribution limits are:

  • Health Care FSA: $3,400
  • Dependent Care FSA: $7,500
  • HSA for Individuals: $4,400
  • HSA for Families: $8,750

Health Savings Account (HSA)

When you enroll in the high-deductible health plan and meet the eligibility requirements, you can open a health savings account (HSA) to help pay for current out-of-pocket health care costs or save for future expenses—even into retirement.

Dental

The dental plan covers preventive care along with additional benefits and services essential for maintaining good oral health.

Vision

Regular eye exams allow your doctor to detect a variety of medical conditions early and help prevent them from becoming major issues.

Flexible Spending Accounts (FSAs)

Flexible spending accounts (FSAs) let you set aside pre-tax funds for health care and dependent care expenses. These contributions aren't subject to federal, state, or payroll tax withholding, so you can keep more of your paycheck.

Employee Support Programs

Assistance is always available for you. The employee assistance program (EAP) services are provided AT NO COST to you and your household. Your EAP is a free, strictly confidential service that includes 24/7 virtual counseling.

Retirement

Saving for your future is a vital part of your financial planning. By participating in the 401(k) retirement plan, you can build a strong financial foundation for your retirement income, which makes it easy for you to save money on a tax-deferred basis.

Life and AD&D

Life insurance provides a payout to your beneficiaries if you pass away, no matter the cause. Accidental death and dismemberment (AD&D) insurance gives a benefit if you die in an accident or suffer a serious injury.

Disability

An unexpected injury or illness can create a financial burden. Disability insurance replaces a portion of your income when you are unable to work.

Supplemental Life and AD&D

Depending on your personal situation, basic life and AD&D insurance might not be enough for your needs. To protect those who depend on you for financial security, you have the option to purchase supplemental coverage.

Accident
